As per the notification the Candidates have to apply through prescribed application format that should be obtain from the Joint Secretary – II, Health care, Human Services and family Welfare Department, Health Secretariat, Gangtok on the production of bank receipt for Rupees 100/- credited under head 0210-OR(Cost of Application form). Application forms shall be available from 17-09-2012 to 27-09-2012.
