When Is the UPPSC LT Grade DV Date 2026?
UPPSC LT Grade Assistant Teacher document verification is scheduled for 25 September 2026 for Science (Male and Female) and 26 September 2026 for Commerce (Male and Female) and Agriculture/Horticulture (Male). How to Download the UPPSC LT Grade DV Form Set
From 23 September 2026, candidates can print the DV form set online. Follow these steps:
- Visit the Commission website at uppsc.up.nic.in.
- On the Home Page, click the link titled "Click here for Advt. No. A-5/E-1/2025, Assistant Teacher, Trained Graduate Grade (Male/Female Branch) Exam-2025 Document Verification".
- Click "Click here to Authenticate" when it appears.
- Authenticate using your OTR number and password or OTP.
- After authentication, the "Applicant Dashboard" appears on the screen automatically.
- Click "Check Latest Status" against the post you applied for.
- Click "Proceed to Print Complete Form Set for Document Verification".
- The online form set is displayed. It is prepared from the details you filled in earlier and includes the application form, certification form (प्रमाणीकरण प्रपत्र), declaration form (देशना-पत्रक) and marksheet details (अंकतालिका विवरण). Print it.
UPPSC LT Grade DV: Documents Required
Candidates must bring the following documents along with the form set. The notice asks for the original and a photocopy of each document unless stated otherwise.

Important Instructions for UPPSC LT Grade DV
- Candidates must be present for document verification on the date and at the time fixed for them.
- Bring original copies of all mark sheets, degrees and other documents related to educational qualifications, along with reservation-related certificates.
- If the documents show that a candidate does not hold the essential qualifications advertised for the post, the selection/candidature will be cancelled.
- The candidates are provisionally successful at this stage, as described in the notice.
