How to Print the UPPSC LT Grade Form Set for Document Verification

Candidates can print the complete form set from the UPPSC website from 09.10.2026. Follow these steps:

  1. Visit the official website https://uppsc.up.nic.in.
  2. On the Home Page, click the link titled Click here for Advt. No. A-5/E-1/2025, Assistant Teacher, Trained Graduate Grade (Male/Female Branch) Exam-2025 Document Verification.
  3. Click Click here to Authenticate.
  4. Authenticate using your OTR number and password or OTP.
  5. After authentication, the Applicant Dashboard appears on the screen.
  6. Click Check Latest Status against the post you applied for.
  7. Click Proceed to Print Complete Form Set for Document Verification.
  8. The online form set will appear, based on the information you filled in earlier. It includes the application form, certification form, declaration form and marks statement details. Print it.

Documents Required for UPPSC LT Grade Sanskrit Document Verification

Candidates must bring the following documents along with the printed form set. Both original and photocopies are required unless stated otherwise.

  1. High School marksheet and certificate (original and photocopy).
  2. Intermediate marksheet and certificate (original and photocopy).
  3. Marksheets and certificates of every year or semester of the Graduate degree, which is the mandatory educational qualification advertised for the post, along with the Shiksha Snatak (B.Ed.) marksheet and certificate (original and photocopy).
  4. Marksheets and certificates of any other educational qualification.
  5. Caste certificate and residence certificate issued by the competent authority, in original and photocopy. For women candidates, the caste or residence certificate is valid only if issued from the father's side, as per Government Order No. 22/39/1982-Karmik-2 dated 17 May 1984.
  6. Certificate for horizontal reservation (ex-serviceman, D.F.F., P.H. and similar categories) in the format prescribed by the Commission, issued by the competent authority (original and photocopy).
  7. No Objection Certificate (NOC) issued by the appointing authority, for candidates who are government servants or employees (original and photocopy).
  8. Two latest passport-size photographs attested by a gazetted officer, and three unattested photographs.
  9. Original affidavit, in case of any difference in name across documents.

Important Instructions for Candidates

  • Be present on the assigned date and time. Attendance is compulsory.
  • Prepare 2 sets of self-attested copies of all educational documents, certificates and other records, and attach them to the printed form set.
  • Sign at the designated place on every page of the form set.
  • Carry 5 photographs in total: 2 attested by a gazetted officer and 3 unattested.
  • Bring original mark sheets, degrees, caste and reservation certificates and all other original documents related to educational qualifications.
  • If the documents show that a candidate does not hold the mandatory qualifications advertised for the post, the selection or candidature will be cancelled.
